The beneficiary of this scholarship will get classroom training, and general life skills through those fields. Tanzania Port Management Authority (TPA) was established through Law no. 17 of the year 2004. This authority inherited and took over the responsibilities of the former Tanzania Harbors Authority (Tanzania Harbors Authority-THA) which was established through the Port Authority Law no. 12 of the year 1977 after the dissolution of the former East African Community and given more responsibilities to own, operate and develop ports on the coast of the Indian Ocean and the Great Lakes.

That Port Law No. 17 of 2004 gives TPA the responsibilities of owning, managing, operating and developing all port areas in Mainland Tanzania. The main purpose of the establishment of TPA is specified in Section (5) of the Act which is to ensure that Tanzania continues to benefit from its geographical advantage including:

Shaping the better management and productivity of the ports located on the coast of the Indian Ocean and the Great Lakes; to ensure there is productivity in the services provided for loading and unloading cargo and passengers; develop, improve and manage port infrastructure; manage port security; and enter into contracts or agreements with any person or institution for the provision of port services.

BMF has been granted an award by Henry Jackson Medical Research International, Inc. Tanzania (HJFMRI Tanzania) through its U.S. Army Medical Research Acquisition Activity (USAMRAA) to support management of non-clinical Human Resource for Health Component under Southern Highland Zone Civilian Project (SHL) implemented in four regions of Rukwa, Katavi, Songwe and Mbeya. Through this award a total of 317 data management staff will be deployed in 213 health facilities to support program activities in the respective regions. Therefore, this job advert seeks for an innovative, self-driven, dynamic, and competent qualified candidates to fill the following vacancies: –

Job Title: Data Clerks (308 Positions)

Specific Task Job Description

Data Clerk will work at facility level to assure timely entry of patient files in health management database for HTS, CTC, Pharmacy Module and RCH, within one day after reception of the file from the clinician and undertake data verification on a routine basis to ensure accuracy and consistency of the data entered, reported, and submitted.

Responsibilities

  • Enter patient information, family information, visits, and laboratory results and other findings into the CTC2 database system, HTS database and pharmacy module.
  • Ensure all the patients’ records are properly stored and monitored according to the National NACP guidelines on storage of confidential client information.
  • Ensure that patients monitoring tools are neatly kept and easily accessible when needed.
  • Ensure all project reports; daily, monthly, quarterly, semi-annual, and annual reports are of high quality and are produced and submitted on time in the introduced data reporting system.
  • Provide data entry support to CHMTs into the DHIS2 including conducting data reviews and validations at facility level.
  • Collaborate with CBHS at community and health facility levels for tracking defaulters, producing list of Missed Appointment and IIT and updating the database accordingly when patients return to care.
  • Provide all the necessary support and assistance on data requests at facility to program staff and Council Staff.
  • Conduct daily, weekly, and monthly tracking of all HIV positive clients from all testing points at facility level ensuring proper documentation of these clients for easy tracking.
  • Actively participate in all QI related activities and ensuring provision of accurate data on monitored indicators by the QI team within respective assigned facilities.
  • Conduct routine analysis of data and display of key set of HIV/AIDS indicator progress on a monthly/quarterly basis and share findings with the Facility/CTC staff for use and for decision making.
  • Conduct monthly data audit. Verify data entered in CTC2 database and that of the paper-based source. Check the completion of CTC2 cards and work with CTC staff to rectify any inconsistencies.
  • Develop, Review and Maintain copies of data collection forms and reports (ensuring accurate updating as needed) and support documentation for data management.
  • Participate in health education activities for preventing HIV transmission particularly among people living with HIV/AIDS attending clinic in CTC /RCH.
  • Work closely with other staff members at CTC and RCH to provide good services to clients attending the clinic.
  • Actively follow up and track patients who have missed their clinic visits daily after the end of every clinic session.
  • Provide data entry support to CHMT into DHIS2 system including conducting data validation at facility and district.
  • Participate in internal Data Quality Assessment with R/CHMT.
  • Conduct monthly data cleaning exercise for respective site/s.
  • Prepare a list of patients who are eligible for HVL on weekly basis from the CTC2 database, share with CTC in charge ensure all HVL sample collected, and results received are documented and entered to CTC2 cards and CTC2 database on timely manner.
  • Ensure that all TPT records (start and completion) are documented into CTC2 cards and database in a timely manner.
  • Ensure continuous back up of the CTC2 database files after every visit (daily, weekly, or monthly basis).
  • Conduct routine data checks on client information entered CTC2 database on a daily/weekly basis). Conduct day to day synchronization of client’s appointment dates through the SMS reminder system.
  • Provide weekly tracking SMS Reminder report to M&E Officer and updating client’s phone numbers into CTC2 database.
  • Ensure that all the information concerning clients at CTC and RCH are always kept confidential.
  • Ensure all Donor reporting requested are prepared and submitted a request by the M&E team or Data Managers in your region.
  • Ensure updated version of CTC2 database is running as per NACP requirements and ensure weekly and weekly export of data to NACP (Macro3) and monthly Portal is done as required including CQI indicators.
  • Ensure clients’ files are arranged in order based on instructions given during the training or order that will reduce time to search for patients’ files when clients return for next visit.
  • Keep records of appointments of the clients and ensure missed appointments are communicated to outreach providers (HBC) and status of clients are updated timely.
  • Print lost to follow up list on weekly basis and share with outreach partners.
  • As part of capacity building to clinician filling out CTC2 cards to be recommended for next training, data clerk will be required to keep record of the clinician that have not filled out CTC2 card properly. Return cards for correction and keep record of the clinician and frequency of files returned.

Experience, Qualifications and Knowledge

  • Diploma in Health records and information management, Records and Archive management, Records Management, Information technology and data management, Health Informatics, Statistics, biostatistics.
  • A minimum of two (2) years’ experience working as a data entry officer in the HIV/AIDS program settings
  • Analytical and problem-solving skills, multi-tasking, and organizational skills.
  • Ability to communicate fluently in both English and Kiswahili.
  • Ability to work both individually and as part of a team with minimal supervision.
  • Ability to maintain confidentiality in all aspects’ Strong attention to detail and organizational skills with the ability to prioritize and multitask Job.

The region lies at about 5° south and 30° east of Greenwich. The region is bordered to the north by both Burundi and the Kagera Region. To the east, it is bordered by the Shinyanga and Tabora regions, to the south by the Rukwa Region, and to the west by Lake Tanganyika, which forms a border with the Democratic Republic of the Congo.

The region’s total area is 45,066 square kilometres (17,400 sq mi), of which 37,037 square kilometres (14,300 sq mi) is land and 8,029 square kilometres (3,100 sq mi) is water.

The region’s total area is just 161 square kilometres (62 sq mi) less than that of Estonia. As of 1998, approximately 20,000 square kilometres (7,700 sq mi) was in forests and 12,000 square kilometres (4,600 sq mi) was suitable for grazing or farming. Kigoma Region is on a plateau that slopes from the northeast at about 1,750 metres (5,740 ft) down to 800 metres (2,600 ft) at the shore of lake Tanganyika. The topography in the north and east is gently rolling hills that gradually become steeper as they get closer to the Albertine Rift margin. The most important river is the Malagarasi, with the Luiche and the Ruchugi being the two other major rivers draining the region.

Job Summary: Laboratory & Supply Chain Officer will be responsible for ensuring that MDH supported and non- supported health facilities meets national standard for related medicines and commodity supply chain management in council by acting as early system warning on 3 key health Logistic systems of Ministry of health Tanzania (Lab, ILS, & TB logistics systems).

Duties and Responsibilities:

  1. Providing strategic, technical and operational support to the day-to-day Logistic activities and capable of identifying potential breakdown in 3 logistics systems and taking corrective actions to avoid service interruption.
  2. EnsuringuninterruptedLabsupplyofcommodities,bycoachingandprovidingMentorship to health facility staff on Lab & TB commodities management practices, record keeping, reporting and ordering (R&R) so that consistent availability of Laboratory Reagents and supplies at health facility level are in place.
  3. Providing TA to districts teams to ensure MDH supported and non-supported sites/districts meet national standards.
  4. 4. Facilitating trainings on good practice in health commodities Logistics systems.
  5. 5. Ensuring no stock out of Lab and HIV/AIDS commodities is reported to HFs in collaboration with DLT Lab manager by:
  •   Reviewing all R&R sent through eLMIS, reviewing annual forecasting and budgeting of needs in HIV/AIDS non-HIV/AIDS commodities including lab commodities during CCHP’s.
  •   Ensuring all HIV related commodities including lab reagent and consumables are adequately stocked at HFs and assist in redistribution of products when necessary
  •   Providing technical assistance to HFs in collaboration with DLTs in forecasting and ordering of Lab commodities from the MSD.
  •   Keeping track of commodity flow from MSD store and facilities and continuously update.
  •   Providing TA to districts/sites to maintain the inventory of laboratory commodities and reagents that are present at both storage facilities and all MDH supported site Labs.
  •   Conducting quarterly logistics data quality assessments (LDQA’s) at least 10 sites per

    district to ensure that all supplies including medicines and laboratory reagents reach intended beneficiaries.

  1. Following up collection of monthly information about the stock at all MDH supported sites/districts, and share with respective key stakeholders
  2. Actively performing supportive supervisions to MDH supported and non-supported HF’s in the region and use these supervisions to strengthen their capacity on issues related to supply chain management.
  3. Performing any other duty as assigned by the supervisor.

Requirements, Education, work experience and skills

  2. Experience in Logistic and Supply Chain systems in Tanzania will be an added advantage.

Job Summary: The Clinical Tracker supports the facility HTS focal person to ensure efficient implementation and reporting of HIV testing services and ART linkage at the facility. She/He is responsible for ensuring HIV positive clients liked into care and treatment services remain in long life ART use.

Duties and Responsibilities:

  1. Documenting client’s appointment schedules in appointment register.
  2. Reminding clients on their appointment dates at least 1 day before the appointment date
  3. Identify patients who missed their clinic visits schedule timely and conduct telephone

    tracking.

  4. Preparing and share with CBHS providers/peers/community tracker list of clients who

    were not found through the phone and those who don’t have telephone for physical

    reminding and tracking.

  5. Ensuring all tracking outcomes are updated into the CTC 2 card and CTC2 database in

    collaboration with clinician/counselor and Data Officer.

  6. Conducting weekly review and ensure all tracking outcome are updated in all tracking tools

and entered on CTC2 database.

  2. Working with counselor and data officer to confirm all transfer out in their respective

    facilities.

  3. CoordinatinghealtheducationprovisiononimportanceofclinicandARVdrugadherence

  4. Supporting the patient adherence to medication and clinic visit schedules.
  5. Recording and report clinic attendance and tracking services daily, weekly, monthly and

    quarterly .

  6. Participating in data management and net loss analysis in respective facility and district.
  7. Participating in QI activities that aimed to increase retention at facility level.
  8. Bringing up any ideas or comments that may contribute to better system of patients follow

up.

  1. At least a Diploma in social work, community development or any other health related field, Certificate of Secondary Education/Social Work with 3 years relevant experience.

  3. Analytical and problem-solving skills, multi-tasking and organizational skills.
  4. Highest level of integrity.

Duties and Responsibilities:

  1. Adhering to nursing ethics as punctuality and cleanliness etc.
  2. Ensuring CTC clinic runs in accordance with national standards.
  3. Ensuring all clients are taken recommended investigations such as HVL, CD4 in

    accordance with guidelines.

  4. Ensuring patients receive appropriate clinical care during a clinic visits, including scheduled

    or unscheduled physician examination and laboratory test.

  5. EducatingpatientsinallaspectsofHIVdiseaseandtreatmentmanagementaswellasbasic

    healthy lifestyles.

  6. Counselling patients with regard to HIV diagnosis, management of medications,

medication and clinic visits adherence, psychosocial issues, and basic health self-care.
